Singapore F-16 Crash: Rare Gyro Failure Identified, RSAF Bolsters Maintenance Protocols

The Republic of Singapore Air Force (RSAF) has concluded its investigation into the F-16 crash that occurred on 08 May 2024 at Tengah Air Base. The probe, conducted by the RSAF with support from Lockheed Martin (the aircraft’s manufacturer) and the Transport Safety Investigation Bureau, pinpointed a rare malfunction of the aircraft’s pitch rate gyroscopes as the root cause of the accident.

Gyro Failure: A Critical Issue

Pitch rate gyroscopes are vital components within the F-16’s flight control system. These sensors constantly measure the aircraft’s rotation around its horizontal axis, feeding this crucial data into the Digital Flight Control Computer (DFCC). The DFCC, in turn, uses this information to precisely maneuver the aircraft. A malfunction in these gyroscopes can provide inaccurate data to the DFCC, potentially causing the pilot to lose control — a highly uncommon occurrence. This incident marks the first reported case of such a failure in the F-16’s long operational history, which began in 1974.

The investigation reassuringly confirmed that the RSAF adhered to all established maintenance protocols for the specific F-16 involved in the crash. However, to further mitigate the risk of similar incidents and prioritize pilot safety, the RSAF has proactively implemented an additional preventative maintenance procedure.

This new procedure will allow engineers to detect potential degradation in the pitch rate gyroscopes at an earlier stage.
